All proceeds from the reduced-rate sports physicals will be donated back to the athlete’s school. The cost is $20 and is due at the time of service. Parental consent must be given for any student under the age of 18. To give consent, please either call ahead or accompany your child to their physical.

For a sports physical in Oskaloosa, use entrance #4. New Sharon Medical Center is located at 112 South Main Street. Final check-in is 15 minutes before close at both locations.

Most schools require that any student planning to participate in athletics has a pre-participation physical. If you have questions about the requirements for your child’s school, please contact your school.
